CERTIFICATION. By typing my name below, I certify that:

• All information provided by me in, and in support of, this application is true and complete and that any and all information I have provided is subject to verification. I understand that any false or misleading statements on this application or concealment or omission of requested facts will result in a decision not to hire me or, if already hired, termination of my employment;

• I understand that my completion of this application form does not mean that there is an available open position nor is Prospect obligated to hire me for any position. In the event that Prospect does hire me:

o I acknowledge I will be an AT-WILL employee of the company during my entire period of employment with Prospect. Being employed “at-will” means that Prospect can terminate my employment at any time for any reason with or without giving me notice and that I may end my employment for any reason at any time. I am also aware that any change to my at-will status is not valid unless it is in writing and signed by the President of the Company;

o I agree to abide by any and all terms of employment stated in this application for employment as well as Prospect’s workplace rules, policies and procedures (and any amendments thereto) and will familiarize myself with the same;

o I agree not to disclose or make use of any of the Company’s trade secrets or other confidential or restricted information either during my employment or after my employment with the Company ends; AND

o I will submit to a medical examination(s) by a Company-appointed physician upon request and will submit to such examination before making any claim against the Company for injuries suffered in connection with my employment.

• I also understand that my completed application will remain on active status for three (3) months from the date it is received by the Company and, if hired, becomes a part of my official employment record. If I am not hired within the active status period, I understand that I must submit a new application to be considered for future open positions.

AUTHORIZATION AND RELEASE. By typing my name below, I authorize Prospect to:

• Conduct a thorough investigation of my background, employment record, and any and all other matters related to suitability for employment.

• Contact the references and employers named herein who are authorized to disclose to Prospect any and all reports and other information related to my suitability for employment, personal or otherwise, without providing me with prior notice of such disclosure. I agree to execute and complete any additional individual Request for Information from Previous Employer authorization forms that may be requested from the Company to accomplish these contacts. I release Prospect, my past and present employers, and all references listed herein from any and all claims, demands and liability which may result from furnishing information to Prospect related to my suitability for employment.

• Release information contained within my official employment record (including this application) to anyone inquiring about my employment or application for employment with the Company upon request.

• If hired, I understand that my employment shall be subject to the worker’s compensation laws of the state in which I am stationed. Accordingly, I waive for myself, my heirs, and representatives all actions at law against Prospect for damages due to my injury or death in connection with my employment and agree to accept the applicable compensation award provided for under state law.

• If hired, I hereby grant the Company the absolute and irrevocable right and permission to use and copyright any photograph or likeness of me for use or publication in any medium, for any purpose, including but not limited to: illustration, advertising, promotion and trade. I release Prospect, its employees, agents, and personnel acting on its behalf to use my photograph or likeness, from any and all claims and demands in connection with the use of such images, including, but not limited to, claims for libel and invasion of privacy.

ACKNOWLEDGMENT OF REQUIRED DRUG AND/OR ALCOHOL TESTING. By typing my name below, I acknowledge that:

• Pre-employment drug screening may be required. I understand that if I am offered employment, my employment may be conditioned on  me taking and receiving a negative result on a pre-employment drug test in accordance with the Federal Aviation Administration’s Anti-Drug Testing Program. Pre-employment drug testing under this program is conducted to determine the presence of marijuana, cocaine, opiates, phencyclidine (PCP), and amphetamines, or a metabolite of those drugs in an individual’s system.

• I also understand that if hired, I will be subject to the Company’s Drug and Alcohol Testing Program which requires all employees to submit to random drug and alcohol testing as well as mandatory drug and alcohol testing in the event there is reasonable suspicion to believe the employee is under the influence of alcohol and/or drugs or in the event of an injury or accident at work. I understand that my employment with Prospect is subject to immediate termination if at any point during my employment I fail to comply with the Company’s Drug and Alcohol Testing Program or if I have a verified positive drug and/or alcohol test reported to the Company.
